Please use the following correct legal name when assisting a client in making a gift through his or her will, living trust, or other estate plan: The Amyotrophic Lateral Sclerosis Association.

A sample provision might read as follows

"I bequeath (here state the specific dollar amount, percentage, residue, or description of real or personal property) to The Amyotrophic Lateral Sclerosis Association, currently headquartered in Calabasas Hills, California, to be used or disposed of as The Association in its sole discretion deems appropriate."

The Amyotrophic Lateral Sclerosis Association's federal identification number is: 13-3271855.

Prior Addresses for The ALS Association

The National Office of The Amyotrophic Lateral Sclerosis Association has been located at the above address since November 1998.

Please note that from 1989 to October 1998, the National Office of The ALS Association was located in Woodland Hills, California. Prior to that date, The Association was located in Sherman Oaks, California.

If you are administering an estate gift that specifies either a Sherman Oaks or Woodland Hills, California, address, please change your records to reflect ALSA's current address listed above.

One final note, The Amyotrophic Lateral Sclerosis Association incorporated in 1985. The Association was formed by the merger of two organizations - the ALS Society of America and the National ALS Foundation. An estate gift that specified either of these two previous organizations is a gift to The ALS Association.
